We would like you to join us as a Wellbeing Collaborative referral partner.

We are developing a range of information access points across the following areas:

• BCP Council Adult Social Care
• University Hospitals Dorset Foundation Trust (Poole Hospital and Royal Bournemouth Hospital)

We are offering them supported signposting to find and access the quality services that you provide in their local community.
