CAN Wellbeing Collaborative

    We connect people to personalised information and support from local charities and community services, so they can live their best possible life at home.  


    Our Partnership

    The CAN Wellbeing Collaborative is a partnership co-creating new ways of working together that benefit everyone:

    ​


    How we will do this

    • Working directly with consenting patients and carers to identify their needs and help them to access community information and support.
    • Building a network of providers and supporting them to become referral ready.
    • Developing engaging information access points in a variety of locations which connect patients and carers to a wide variety of quality assured community support.
    • Integrating volunteering options to support people in finding the information they need, at a place and time that is right for them.
