Help us shape the future of VCSE and Statutory Service collaboration to keep people safe and well at home for longer.

Home First is a new partnership approach to provide more community-based care in Dorset and reduce unnecessary hospital stays. Since September 2020, NHS and social care services in Dorset have been teaming up to help people out of hospital more quickly and recover in the comfort of their own homes wherever possible.

Community Action Network and Dorset Community Action are hosting a project to develop the role of the Voluntary, Community and Social Enterprise Sector in hospital discharge and in the commissioning of personalised care and support for people leaving hospital.

This means that as soon as a person is ready to leave hospital, they will be discharged home – or to a local care home or community hospital, if that is more appropriate – and assessed to see if they require additional support to aid their recovery.

We are currently gathering evidence to support the need for change in the way VCSE services are working with our health and social care partners. We know that many small organisations and voluntary groups already provide practical help and wellbeing support for people in communities across the whole of Dorset so this is the first of a three stage process to co-design solutions to improve services. We want to hear about

  • what is working well for you
  • the wellbeing issues you are regularly dealing with in your community
  • any problems you have in working with statutory partners
  • any organisational support you might need to expand in this area
